Title: On call Nurse
Job summary: The primary responsibility of LPN on-call to provide medical services and under the supervision of a (RN)Lead Nurse.
Job functions:
On-call
- Provide medical/health-related consultation to staff during regular or after hours of operation through on-call. Determine the need for services including assessments, symptom management, administration of PRN medication, directing staff to seek outside care such as at an Urgent Care center and other directives to the call.
- Maintain an accurate clinical record through the timely documentation of all client specific telephone calls received by entering detailed notes regarding the specifics of the call and instructions provided to staff into the client’s electronic health record (Therap).
- Maintains regular communication with all members of the interdisciplinary team, alerting physician and Lead Nurse (RN) to any changes in the client’s condition or needs and potential problems or issues that may be identified through nursing calls.
- Gather consults and transcribe in Therap system.
- Complete medication reviews (med closets) at group homes
- Observe new hire medication pass administration and verification of competence
- Review of medication administration records and physician orders
- Input new medication on EMAR
- Medication administration training (PRN)
- Observe random medication passes
- Observe new hire insulin pass administration and verification of competence
- Monitor and report monthly, quarterly, semi-annual blood pressure reports
- Review of medication errors and provide on-site education to staff with deficiency in medication administration
- Other duties as assigned by the RN or Executive Director
Additional Responsibilities: Other responsibilities and duties may be assigned as needed.
Schedule:
On-call: every other weekend starting Friday 5pm – Monday 9am
Pay: $23 – $25 per hour

Education: Licensed Practical Nurse with a valid NC nursing license and NC driver’s license. DD or other job- related experience preferred.
Pay: $25.00 – $30.00 per hour

QIDP
Candidates must have at least two years of work experience writing and executing person centered goals and developing effective strategies for successful implementation. Work experience implementing the NC Innovations Waver and/or ICF/IDD standards and supervisory experience preferred.
Must have a BA/BS in Human Services and 2 years of FT post graduate experience.
Job Type: Full-time
Pay: $42,000.00 – $44,000.00 per year
