Title: Lead Nurse
Job summary:
The Registered Nurse plans and evaluates the medical services of ASMC group homes, and is responsible for the oversight of the medical care of all the clients. The RN works closely with the clients’ families, physicians, on-call nurses, LPNs, QPs, other IDT members, staff and community partners. The RN provides training/education to the families, the clients, ASMC employees and staff of contracted agencies.
Essential Duties:
- Provides above standard nursing care by tracking, planning, organizing and directing paraprofessional staff and professional staff on the healthcare of each individual.
- Evaluates and monitors resident’s health status. Develops the care plan to address the client’s medical/health needs.
- Provides prompt consultation in person and by telephone during regular business hours and when on-call evenings and weekends.
- Ensures that adequate nursing logs (Therap) are completed and timely follow-ups are occurring from on-call nursing.
- Implements new physician’s orders and reviews current and standing orders promptly for accuracy.
- Adheres to state/federal regulations and company standards regarding medical care.
- Trains AMSC and contracted DAP staff, who may need to provide client specific medical services to ASMC clients, such as taking blood pressure, Epi- Pens, seizure management.
- Evaluates the administration of medication performance of staff through observations, reviewing of medication administration records and error report, and consults with Residential Director on appropriate action to be taken.
- Maintains confidentially of all PHI/ePHI and be sensitive in handling delicate situations.
- Serves as member of the interdisciplinary team and participate in plan meetings.
- Reviews medication error and notifies physician of medication error.
- Reviews accident/incident reports involving client injuries and make appropriate and timely recommendations to QIDPs when needed.
- Reviews monthly Medication Administration Records (EMAR/MAR), controlled drug records, seizure and menses records for each client.
- Attends quarterly psychotropic drug reviews and HRC meetings.
- Coordinates and ensures that all prescribed medical/health care recommendations are followed and in the least restrictive manner possible.
- Prepares annual and quarterly medical/health care report of each client.
- Conducts quarterly direct physical examination of the client’s body.
- Assists in reviewing and revising medical related policies and procedures.
- Establishes open lines of communication with contracted agencies and community partners and consistently take follow up action when needed.
- Maintains the standards of accurate and complete documentation and reporting in Therap.
